ALBEMARLE COUNTY, Va. (CBS19 NEWS) — A cancer survivorship plan is an 8- to 10-page document that provides survivors with items like their treatment history, possible side effects, what medications they need to take and when, when they need to follow up with screenings, how often they should see their oncologist, or when they should see the survivorship clinic nurse practitioner.

“It also gives them some tips and tricks of, they’re still going to have some late side effects from treatment, how do we deal with those, its also going to help them with their emotional needs, and what resources are out there for them?” said Danielle Schrader, Blue Ridge Cancer Program manager.

The survivorship care plans are given to survivors about three to six months after they finish treatment. It is tailored to the patient by the nurse navigator, who meets with the patient and goes over the packet …
